What to Bring

If this is your first appointment, Oak Path staff will ask you to fill out and return a few documents ahead of time.

If you are filing with insurance, please bring your insurance card as well as a method to pay for your co-payment or other out-of-pocket expenses.

If you do not wish to file with your insurance, or if you lack insurance, please don’t allow that to prevent you from reaching out to us, as we also accept self-paying clients.

We accept payment by cash, check, and all major credit cards.

What to Expect

If you have an in-person appointment, please arrive at our office ten to fifteen minutes early.

Our address is 4310 Thermal Avenue Suite C, Midland, NC 28107. Thermal Avenue is located extremely close to the intersection of Hwy 24/27 and 601. Our office entrance is located in the brick building on your right as you turn on Thermal Avenue from Hwy 24/27. You should be able to see the Oak Path Counseling Group sign beside a gray steel door, which is our entrance. You may park in any space available.

Prior to entering, please give us a call at (704) 888-1616 to let us know you have arrived. Please enter through the gray steel door and you will see our waiting area immediately on your left.

Please have a seat and wait for a staff member to call you back. If you were unable to get in touch with a staff member over the phone upon arrival, please ring the bell indicated on the small table in the waiting area and make yourself comfortable. We will be with you shortly.

If you have trouble finding our office, or if you have any additional questions, please call us at (704) 888-1616.

If you have a digital appointment, your counselor will email you a link shortly before your appointment. This link will send you to a virtual waiting room. When your counselor is ready, they will start the appointment.

Your counselor will conduct your appointment in private, and your conversation with your counselor is completely confidential. For more information about our confidentiality policy, please review your intake packet.

Missed and Cancelled Appointments

If you need to cancel your appointment, we understand. Please let us know by calling us at (704) 888-1616. However, we do ask that you try to let us know at least 24 hours ahead of your scheduled appointment if you need to cancel. We do this in order to keep our counselors available for other clients who may be able to fill open slots from cancelled appointments.

Please note that any cancellations or rebookings made without 24 hours’ notice may be subject to a $50 cancellation fee. However, we do our best to take emergency situations and extenuating circumstances into account.
